Q. What should I expect at my first physical therapy session?

At Momentous Sports Medicine, your first Physical Therapy session will give you an hour to consult with your Physical Therapist about what brings you to physical therapy and what will be done to solve your issue. You should bring loose fitting or workout clothes with you as we will need to access the body part(s) in question. If you have a lower body injury, please bring shorts with you. We will perform a full assessment of your body to gain an understanding of what is causing your pain. Once determined, we’ll have a discussion about your issue and the plan of care to resolve it. We provide treatment during this first session to start addressing your pain, along with a personalized home exercise program to help maintain the progress we make during the session. We rarely tell people to stop training or being active, so we will also discuss modifications to allow you to continue training, if necessary.

Q. Do you take my insurance?

At Momentous Sports Medicine, we are out-of-network with all insurances. However, upon request we can provide you with an invoice (Superbill) for you to submit a claim to your insurance for reimbursement based on your out-of-network benefits. Many out-of-network benefits reimburse a part, if not all of the services billed once your deductible has been met, but it is always good to check your benefits. You can also use your flexible savings account (FSA) or health savings account (HSA) to cover your Physical Therapy visits.

Q. Why should I work with Momentous vs. an in-network provider?

At Momentous Sports Medicine, we provide world class physical therapy services to all of our athletes, because we believe everyone is an athlete. Everyone needs to move and no matter what that movement is, you need to do it pain-free and efficiently. We provide one-on-one, personalized treatment to all of our patients, with no assistants or techs for a full 30 or 60 minute session. We never double-book or overlap appointments. We listen to you and provide you what you need to reach your goals. Your entire experience will be patient centered, on you.

Our goal is to help you return to all activities pain-free as efficiently as possible and not have you return for the same issue a few months later. We give our undivided attention to you for 30-60 minutes, focusing on your whole body using movement analysis, manual therapy, neuromuscular re-education, and performance training to address your pain and where your pain is coming from to prevent it from recurring. Using this approach tends to require us to need to see our patients for less sessions than an in-network provider. This means that you get out of Physical Therapy and back to activity sooner, leading to cost and time savings with a better recovery outcome.

As an out-of-network provider, we are able to provide the best possible treatments for you, and not be restricted to only what insurance will cover. For instance, if a patient only has pain with running, but is able to function pain free otherwise, insurance will stop covering sessions. However, as an active person, running may be what you need to be treated for. As a sports Physical Therapist, we’re here to help you do all of your activities pain-free, and that includes training, working out, and playing sports.

Q. Why should I get a membership?

Our Momentous Membership is a great way to take control of your health and prevent injury. We generally recommend memberships for people who are healthy and active and are looking to improve performance and reduce their risk of injury, or if you’re a patient and you’re returning to full activity. Having a monthly session keeps you accountable and having a regular check-in can be helpful to catch something minor before it turns into something debilitating. Q. What is the difference between the two types of memberships?

Our monthly membership is $185/month and you can cancel at any time. It includes all the benefits of a Momentous Membership, however you can only get the 10% off packages if you’ve been a member for at least 3 months.

Our annual membership is $150/month and is a 12-month commitment. You’ll be charged $150 once a month and receive all the benefits of a Momentous Membership. You save over $400 by being an annual member!

Q. Does my membership monthly wellness session roll over month to month?

Our Momentous Membership offers a free monthly wellness session, but it does not roll over if you don’t use it that month. The reason behind this is because the monthly session is to help you stay on top of your health and help you be accountable in coming in regularly to address little aches and pains, or prevent injury. These sessions aren’t as effective if you don’t use them once per month.

Q. What is your Tax ID Number (TIN)?

For patients submitting superbills to their insurances, we often get asked what our Tax ID Number (TIN) is. If you’re looking at your superbill, scroll all the way to the bottom and our TIN is the 9-digit number next to our phone number and is formatted as: XX-XXXXXXX. If you’re still having trouble finding it, please call us!
